Application Procedure

You will need a free Adobe PDF Reader to download the forms in this section.  If you have not installed the free Reader you may obtain it from the Adobe Reader website. If you have problems filling out the PDF forms, you may use the Word format to fill out the forms.

You may download and fill out any of the PDF forms below and email the completed form as noted below. 

Alternatively, you may wish to print out these forms and fax or mail them to BICWM.

1. Application
Complete the Missionary Development Program Participant Application.

3. Initial Contact
You will be contacted by a person from the Missionary Development Team to conduct a telephone interview as a follow-up to your written application.

4. Review
The Missionary Development Team reviews all participants in the Missionary Development Program. Your pastor will receive a confidential reference form which is to be returned to the MDP Office. Then you will be contacted following this review process as to your acceptance into the program.
